OUTREACH SERVICE
Outreach Service
The Centre has an outreach service where carers can obtain information and support in their own communities.
Every 1st Wednesday of the month at the Bransholme Health Centre, Goodhart Road, Bransholme, Hull, HU7 4DW between 9am to 12 noon.
Every 3rd Wednesday of the month at the Clifton Medical Centre, 263-265 Beverley Road, Hull, HU5 2ST between 8.30am to 11.30am.
Carers can visit with the person they care for and leave them in the care of a support worker during their appointment.

Outreach Survey
The Centre has been developing its Outreach Service over the last year and we would like to ask you to complete a simple survey on the Outreach Service, even if you have never used it.
The service has held drop-in support sessions in a number of health centres around Hull enabling carers to pop-in to gain information and support on a number of carer related issues.
This survey will take less than 5 minutes to complete and no personal information will be asked of you.
